How about everybody holds their hands behind their back and then you start singing
“Where is thumb-kin? Where is thumb-kin?”
And first you bring out your right hand with thumb up
“Here I am”
And the left hand with thumb up
“Here I am”
Shake the right thumb
“How are you today sir?”
Shake the left thumb
“Very well, I thank you”
Move right thumb behind your back
“Run away”
Move left thumb behind your back
“Run away”

And you would repeat it for every finger on your hand. We used to play it in kindergarden in the Bronx
